There are at least two different types of boxes for Melody Blaster. The
traditional book-style box, and the flip-top box, which was common late in the
life of the Intellivision at Mattel. It seems odd that such a late release
would ever have had a book-type box.
In addition to the two box styles, the backs of the two different styles
list different 'made in...' text in the lower left corner.

not yet been carried out. The page the errata slip addresses is shown here.
